Welcome to our web site! We look forward to meeting you and working with you to achieve your orthodontic goals. Prior to your first visit with Dr. Chadwell we encourage you to download the Patient Information/Health History Form and complete it as accurately as possible. Print out the form and bring it with you along with your insurance card(s) and any other information you believe may be important.

Your First Visit

Your first visit to Chadwell Orthodontics starts with a warm welcome and a tour of our office. During your appointment Dr. Chadwell will review your medical/dental history, evaluate your orthodontic concerns, discuss the possible treatment options and address any questions you may have regarding treatment. If treatment is indicated we will discuss your treatment appointments, scheduling and payment options. This initial visit usually takes 45 minutes to one hour.

Diagnostic Records

Diagnostic records are taken to determine the best course of treatment. Complete diagnostic records typically include digital scans of the teeth, photos of the face and teeth, and radiographs (X-rays). This information is used, along with the initial clinical examination findings, to develop a custom treatment plan for each patient. These records are also helpful in tracking the progress of treatment as the teeth move under orthodontic care.


After Dr. Chadwell has evaluated your records, we will discuss your treatment with you in detail and answer any questions you may have regarding your case. We will also review our payment options with you and have any necessary paperwork signed and reviewed.

Regular Appointments

The appointment when you get your appliances (braces, expanders, etc.) usually takes from one to two hours. You will then see us at regular intervals 4 to 8 weeks apart for an adjustment, which typically takes 20 to 40 minutes. Our regular office hours are Tuesday through Thursday with alternating Mondays and Fridays from 8:00 AM to 5 PM. Wednesday we schedule patients from 7:00 AM to 4 PM. Patients are seen by appointment only. We make every effort to be on time for our patients, and ask that you extend the same courtesy to us. Since every appointment requires preparation, coordination and timing on our part as well as yours, please cancel only if it is an emergency. If you cannot keep an appointment, please notify us immediately so we may accommodate another patient. Please keep in mind that it can be difficult to reschedule you since most appointments are made 4 to 8 weeks in advance.

Payment & Office Policies


The cost of treatment depends on the severity of the patient’s existing condition. You will be able to discuss fees and payment options before treatment begins. We have various payment plans available to suit your budget. We also accept assignment from most insurance plans, and file the necessary paperwork to the insurance company. We work hard to make orthodontics affordable!


In an effort to keep orthodontic fees down while maintaining the highest level of professional care, we have established the following financial policy:

  • If payment is made in full at the onset of treatment, we offer a bookkeeping courtesy.
  • To fit your individual needs, financial arrangements can be made to extend your payments over a period of months.
  • For your convenience, we offer direct-debit from checking and savings accounts as well as payment by VISA, MASTERCARD and DISCOVER.


If you have insurance, we will help you determine the coverage you have available. We ask that you assign your insurance benefit to us. The balance will be arranged for you to pay (see above). Professional care is provided to you, our patient, and not to an insurance company. Thus, the insurance company is responsible to the patient and the patient is responsible to the doctor. We will help in every way we can in filing your claim and handling insurance questions from our office on your behalf.
